Frank Cotton discovers a puzzle box that leads him to a realm where pain and pleasure are intertwined, ultimately summoning the Cenobites. After experiencing their torment, Frank escapes but requires the life force of others to regain his physical form. When his family moves into his former home, his brother's wife inadvertently opens the box again, leading to a series of horrific events as the Cenobites seek to reclaim Frank and deal with those who have disturbed their domain.
